Lower blepharoplasty is a surgical procedure that addresses protruding fat and sagging skin and muscles in the lower eyelid. If sunken areas are present, the protruding fat is repositioned downwards to create a flat surface, rather than simply removing it.
This procedure is suitable for cases where under-eye fat protrudes and casts shadows, or where the skin under the eyes sags and wrinkles are deep. Non-incisional fat repositioning via the conjunctiva is recommended for younger patients with minimal skin sagging, while incisional lower eyelid surgery is recommended for middle-aged and older patients with accompanying skin sagging.
Access is made through the area below the eyelashes (skin incision) or the conjunctiva (non-incision). Protruding fat is removed or repositioned to sunken areas, and sagging skin and muscles are smoothed to create a flat surface. The incision method allows for skin reshaping, while the non-incision method leaves no scars.
The non-incisional method allows for faster recovery and less swelling, while the incisional method requires stitches to be removed after 5 to 7 days. Initial bruising and swelling subside over 2 to 3 weeks, and the scar becomes hidden along the eyelash line and gradually fades.
Repositioning is preferred because excessive fat removal can actually make the eyelids appear sunken. In the incisional method, overcorrection of the skin or muscle can cause the lower eyelid to be pulled, so precise adjustment is required.
Under-eye fat repositioning is a method of lower blepharoplasty (usually non-incisional). If skin sagging is also present, an incisional lower blepharoplasty is performed to address the skin as well.
Structural dark circles caused by fat or hollowness can be significantly improved, but pigmented or vascular dark circles may require separate treatment such as laser therapy.
