They seem to recommend at least 2-3 days off. I had mine done on Friday, so I rested through the weekend and it was fine! Yes, it's decided after the eye exam. Also, I recommend getting consultations at a few different clinics. There are many "factory-style" eye clinics these days, so go somewhere that pays close attention to you. I was researching LASIK and saw that it's important to check for "Double Expert Doctors," and Gangnam Clear Eye Clinic seemed suitable. You should also try Bright Sharing Eye Clinic. Someone I know had Smile LASIK done here, and they recommended it, saying it's famous as a specialized vision correction hospital in the Gangnam area and that many people go there.
