How should I define the scope of consent when a doctor says they will post photos taken at the hospital after surgery for marketing or social media?

It's safer to specifically state the purpose, medium, duration, scope of anonymization, whether commercial use is permitted, and the withdrawal/deletion procedure in the consent form, and to separate consent for providing the original content and secondary use into distinct agreements.
